You will receive confirmation from Health iPASS via email or text, with a link to fill out your patient intake forms.
You will also receive an email from Exscribe, our Electronic Health Records partner, with a link to complete your medical history forms ahead of your appointment.
If you haven’t filled out your patient intake forms and medical history remotely, we will provide a tablet during your visit to complete these forms.
If you use Health iPASS for contactless check-in, please let the front desk know when you’ve arrived at the office.
Our required patient intake forms are made available online. This allows you to print and complete them before your appointment.
